Ben Lomond Fire Protection District
The following disbursements are unclaimed by the listed payees and held by the County of Santa Cruz Property Tax Fund. If you have a claim against these funds, please check the BLFPD web site at benlomondfd.com for instructions and claim forms or call (831) 336-5495. Funds not claimed by July 31, 2017 become the property of the BLFPD. This notice and its contents are in accordance with California Government Code Section 50050.
Reissued Warrant
To file a claim, please complete Exhibit 2 (Unclaimed Money – Claim Form) with the following information:
- claimant’s name
- warrant number
- proof of identity
- the amount of the claim
- the grounds on which the claim is founded
Please bring the completed Unclaimed Money – Claim Form to the Ben Lomond Fire Protection District Administrative Office at 9430 Why 9, Ben Lomond between 9:00 a.m. and 4:00 p.m. We will need proof of identity and payment of the reissuance fee to process the claim.
